There are 3 identities for the same person, at the same entity, with different Year-Month of birth. They are:
(born 14-Jun-1965, this is correct based on annual returns for 2010 and 2015, I haven’t checked the rest)
(born 14-Feb-1965, incorrectly stated on the appointment form)
(born 14-Jun-1968, which is a transcription error from the appointment form).
I suggest that they be combined into the first identity above.
